This guide explains the "Consents and Agreements" feature within the Mynd platform. These are crucial documents that clients may need to review and sign, particularly during onboarding, ensuring clarity and compliance. This feature is distinct from the main "Forms" module, which is used for more general data collection like intake forms.
"Consents and Agreements" are primarily documents requiring a signature, streamlined for the onboarding process and other key moments in the client journey.
Mynd provides several standard consent forms available to all users:
Mynd Privacy Policy:
Details how Mynd handles data and privacy.
This form is non-modifiable by the practice but can be viewed.
It is included in every onboarding process.
Consent for Telehealth:
Outlines terms for using Mynd as the telehealth hosting platform.
This form is also non-modifiable but can be viewed.
It is included in every onboarding process.
AI Note-Taking Consent:
Explains how AI note-taking features work within the platform and seeks client consent for its use.
This is an optional consent form. Practices can choose whether to present it.
Practices have control over additional consent forms:
Informed Consent for Therapy (Template):
Mynd provides a generic template for informed consent for therapy.
Practices can use this template as-is or modify it to suit their specific requirements.
Custom Consents and Agreements:
Practices can create and upload their own custom consent forms.
Practices can manage and configure consent forms in Client Settings. Here, they can:

View standard Mynd forms (Privacy Policy, Telehealth).
Modify the "Informed Consent for Therapy" template.
Add new custom consent forms.
Configure onboarding requirements:
Require during onboarding: If selected, the client must sign this form to complete their onboarding and access the platform.
Optional during onboarding: The form will be presented to the client during onboarding, but signing it is not mandatory to proceed. They can still complete onboarding and access the platform without signing.
Practices can also choose not to present a specific consent form during the initial onboarding. This is useful in scenarios where a client might join for an initial consultation but only needs to sign a therapy consent form once they decide to book ongoing therapy sessions. The form can then be sent to the client for signature at the appropriate time.
Consents and Agreements: Designed for signature-based documents essential for compliance and terms agreement, typically integrated into the onboarding flow or sent at specific touchpoints. They appear by default when a client is invited to the platform or books via a public booking link.
Forms Module: Used for building more elaborate data collection forms, such as detailed intake questionnaires, assessments, or surveys. If a practice has a very complex consent document that requires extensive information gathering beyond a simple signature, they could technically build it using the Forms module and send it to the client separately.
Streamlined Onboarding: Integrate essential consent collection directly into the client onboarding process.
Compliance: Ensure necessary legal and ethical agreements are signed and recorded.
Flexibility:
Use standard Mynd forms for common needs.
Customize templates or create entirely new forms for specific practice requirements.
Control whether forms are mandatory or optional during onboarding.
Clarity: Provide clients with clear information regarding privacy, telehealth, AI usage, and therapy terms.
